Thomas J. Seefred Trust Scholarship

History: 
           The Thomas J. Seefred Scholarship was established in 2005 with funding generated by the Thomas J. Seefred Trust.  Its purpose is to award scholarships for college students between the ages of 18 and 25 who suffer from Juvenile Diabetes and are pursuing a bachelor’s degree.
 
Award: 
          Each recipient maybe awarded up to $3,000 to be used for expenses such as tuition, fees, room and board during the academic year. Please note that preference will be given to residents of Mahoning, Trumbull, or Columbiana counties.   

Eligibility:  Applicants must meet ALL of the following criteria: (Ohio residents only)
                  - 18-25 years old at time of application
                   - Diagnosed with Juvenile Diabetes
                   - Successfully completing a standard curriculum leading to a bachelor's degree at an accredited
                      junior college, college or university.  This intends the individual be attending college on a full-time
                      basis.  Current high school students are not eligible.                   
                    - Must be in good academic standing
Selection/Evaluation Criteria: 
                               Awards will be based on academic promise, personal statement, recommendations, and
                                financial need
 

Requirements: Completed application
                          - Proof of enrollment - an official dated transcript from the college indicating
                            current enrollment and GPA
                           - Physician’s statement on letterhead noting diagnosis and treatment
                           - One (1) letter of recommendation from a non-relative
                           - Personal essay (500 words or less describing the impact juvenile diabetes has had on you
                              and why you feel  you should receive this scholarship.
                           - Current income tax statements for parents and student.
                               - The recipient shall be required to maintain records of expenditures and proof thereof.                           - Please provide 4 copies of all documents.

Previous Recipients:
           Previous recipients must include an official dated transcript from the college or university, current income tax statements from you and your parents, and the completed and notarized application.  You do not need to have a doctor's letter, recommendation or personal essay since they are already on file.
 
Deadline:  Applications must be postmarked by April 1 to be considered for the upcoming school year.   No late applications will be considered. 
 
Notification:  Scholarship recipients will be notified via mail by July 1.   Only those selected to receive scholarships will be contacted.
