Seefred Scholarship was established in 2005 with funding generated by the Thomas J. Seefred Trust. Its purpose is to award scholarships for college students who have been diagnosed with Juvenile Diabetes and are pursuing a bachelor's degree Award: Each recipient may be awarded up to $3,000 to be used for expenses such as tuition, fees, room and board during the academic year. Eligibility: Applicants must meet ALL of the following criteria:
- 18-25 years old at time of application - Must be in the process of completing a standard curriculum leading to a bachelor's degree at an accredited junior college, college or university. - Current high school students are not eligible. - Must be in good academic standing. New applicants - please provide 3 copies of each of the following documents
Official dated transcript from college or university providing proof of enrollment Physician's statement on letterhead noting diagnosis and treatment Letter of recommendation from a non-relative Personal essay (500 words or less) describing the impact juvenile diabetes has had on you and why you should be considered for this scholarship Current income tax statements from parents and student (do not include extra schedules and forms - the top two pages are sufficient) Please provide three (3) copies of all documents. *Recipient is required to maintain records of proof of expenditures.
